Practice Closure
I will be closing my urology practice in Sarnia effective June 27, 2025
Appointments
Unfortunately, I do not have a successor taking over my practice. All patients with pending appointments will be contacted regarding next steps as some may be rescheduled based on clinical urgency.
Your appointment may be moved up or offered as a telemedicine consultation
If ongoing urological care is required beyond my closure date, I will try to refer you to another urologist if available.
If your condition can be safely managed by your primary care provider, you will be informed and discharged back to their care.
Health Records
In most cases, relevant investigations and clinical notes have already been sent to your referring provider (usually your primary care provider). Your medical records will continue to be securely stored in accordance with PHIPA regulations.
If you need a copy of your medical chart, you have the following options:
Hospital records: Contact Bluewater Health Health Records Department for hospital-based investigations and procedural notes.
Office records: Upon request, my office can provide paper or electronic copies of your chart. Please note that an administrative fee applies, in accordance with Ontario Medical Association policy.
